TO REGISTER

• Registration must be completed by a parent or legal guardian.
• The following materials are required:  license or Mass. ID, car registration or utility bill with current street address, and if a new player, copy of birth certificate. 
• The child must be at least 4 years old by May 1st to meet minimum age requirement for T-Ball.

We will hold an additional registration on Feb. 5th at the Salem Council on Aging, located at 5 Broad St., from 6 - 8pm.  FEES WILL BE INCREASED FOR ALL LEVELS AT THIS REGISTRATION.
